Tolec
Description:
Tolec (Ketorolac Tromethamine) is a member of pyrrolopyrrole group of non-steroidal anti-inflammatory drug (NSAID) that exhibits analgesic, anti-inflammatory and antipyretic activity. It has more pronounced analgesic activity than most NSAID’s.

Pharmacology:
It inhibits synthesis of prostaglandin and is a peripherally acting analgesic. It is highly protein bound and is largely metabolized in liver. The products of metabolism and some unchanged drug are excreted in the urine.

Indication/Use:
Tolec is indicated for the short-term management of moderately severe acute pain, usually post-operative pain. Relief of pain associated with surgical procedures such as major abdominal, orthopedic, dental or gynaecological surgery, acute & chronic musculo-skeletal pain, renal colic, cancer pain.

Side Effects:
Gastrointestinal: Nausea, vomiting, dyspepsia, diarrhoea, peptic ulcer, haemorrhage, perforation, liver function abnormalities.
CNS/ Musculo-skeletal – Drowsiness, dizziness, sweating, convulsions.
Renal- Acute renal failure, flank pain, haematuria.
Other – Hypersensitivity reactions (anaphylaxis, bronchospasm laryngeal oedema, hypotension, flush, rash) oedema, purpura, myalgia, postoperative wound haemorrhage.

Contraindication:
Tolec (Ketorolac Tromethamine) is contraindicated against patients with a previously demonstrated hypersensitivity to NSAIDs and any of the components of Ketorolac Tromethamine. Moreover,the patient with the asthma, nasal polyp, angioedema, peptic ulcer and bleeding, bleeding disorders are contraindicated for this drug.

Use in Pregnancy & Lactation:
Avoid in pregnant and lactating women.

Over Dose:
Symptoms of overdose may include stomach pain, difficulty breathing, and blood in stools.

Precaution:
Use with care in the elderly patients with a history of gastrointestinal disease, those with asthma, cardiac, renal, hepatic & allergic diseases & those who are hypovolaemic. Normally it should not be used in severe renal impairment. NSAIDs have been associated with renal disease. It inhibits platelet aggregation & prolongs bleeding time.
Caution should be used when haemostasis is critical. Monitor prothrombin time of patients on oral anticoagulants. Salicylates reduce plasma protein binding of Ketorolac. Renal clearance of Ketorolac is decreased by probenecid. It reduces the diuretic response of frusemide. Concomitant use of other NSAIDs is not recommended.
