Alagra
Description:
Fexofenadine Hydrochloride (Alagra) is an Antihistamine with selective peripheral H1-receptor antagonist activity. Fexofenadine does not appear to cross the blood brain barrier.

Pharmacology:
Fexofenadine (Alagra) blocks the H1-receptor & thus prevents activation of cells by Histamine in GI tract, large blood vessels & bronchial smooth muscle. This leads to relief of the allergic symptoms. Unlike most other antihistamines, Fexofenadine does not cross Blood Brain Barrier, and therefore, does not cause drowsiness.

Indication/Use:
Fexofenadine ( Alagra) is indicated for relief of symptoms associated with-
Seasonal allergic rhinitis such as sneezing, rhinorrhea, pruritus and lacrimation
Perennial allergic rhinitis
Urticaria

Dosage Guideline:
Age group

30 mg twice daily or 60 mg once daily 60 mg twice daily or 120 mg once daily or 180 mg once daily with water
30 mg (5 ml) twice daily
60 mg once daily is recommended as the starting dose 30 mg (5 ml) once daily is recommended as the starting dose
Children 2 to 5 years

30 mg (5 ml) twice daily

Side Effects:
Fexofenadine is generally well tolerated. The most common side effects are headache, fatigue, dizziness or drowsiness and nausea. In rare cases, rash, urticaria, pruritus and hypersensitivity reactions such as angioedema, dyspnoea, chest tightness, flushing and systemic anaphylaxis have been reported.

Contraindication:
Fexofenadine is contraindicated in patients with a known hypersensitivity to Fexofenadine or any of its ingredients.

Drug Interaction:
Plasma concentrations of Fexofenadine have been increased when given with Erythromycin or Ketoconazole. Antacid containing Aluminium and Magnesium Hydroxide have reduced the absorption of Fexofenadine. Fruit juices including grapefruit may reduce the bioavailability of Fexofenadine and use together should be avoided.

Use in Pregnancy & Lactation:
Fexofenadine is pregnancy category B2. There are no adequate and well controlled studies in pregnant women exposed to Fexofenadine alone or through the administration of Terfenadine. Fexofenadine should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known if Fexofenadine is excreted in human milk. Because many drugs are excreted in human milk, caution should be exercised when Fexofenadine is administered to a nursing woman.

Precaution:
The carcinogenic potential of fexofenadine was assessed using terfenadine studies with adequate fexofenadine exposure (based on plasma area-under-the-concentration vs. time [AUC] values). No evidence of carcinogenicity was observed in an 18-month study in mice and in a 24-month study in rats at oral doses up to 150 mg/kg of terfenadine (which led to fexofenadine exposures that were approximately 3 and 5 times the exposure at the maximum recommended daily oral dose of fexofenadine hydrochloride in adults [180 mg] and children [60 mg] respectively).
