Presentation
Calbeta-25 tablet: Violet, barrel shaped, scored tablet; each tablet contains Amlodipine Besilate BP equivalent to Amlodipine 5 mg and Atenolol BP 25 mg.

Indications
The combination is indicated for the treatment of hypertension and chronic stable angina.

Dosage and administration
In Adults: The recommended dosage is one tablet daily. If necessary, the dosage may be increased to two tablets daily or as advised by the physicians, The dosage however should be individualized. Use in children Not recommended.

Patients with hepatic impairment: Caution may be necessary in the use of the combination in patients with severe liver damage because of prolongation of the elimination half- life of amlodipine.

Patients with renal impairment: The combination can be used in patients with renal impairment. However, caution may be necessary if the creatinine clearance is less than 30 ml/min because of possible reduction in the excretion of unchanged atenolol.

Contra-indications, warnings, etc.
Contra-indications: Amlodipine is contra-indicated in patients with a known sensitivity to dihydropyridines. Amlodipine should not be used in cardiogenic shock, clinically significant aortic stenosis, unstable angina (excluding prinzmetal’s angina). Atenolol is contra-indicated in patients with second degree or third degree heart block and previously known hypersensitivity to the drug.

Precautions: The safety and efficacy of amlodipine in hypertensive crisis has not been established. Special care should be taken with patients whose cardiac reserve is poor. Atenolol may be used with caution in patients with chronic obstructive airways disease.

Use in pregnancy and lactation: The combination should be used during pregnancy only if the expected benefit outweighs the potential foetal risk. Nursing mothers should not use the combination. If its use is considered necessary, breast-feeding should be stopped.

Drug interactions: Disopyramide: Atenolol reduces the clearance of disopyramide by 20%. Additive negative inotropic effects on the heart may be produced. Ampicillin: Ampicillin at doses of 1 g and above may reduce atenolol levels. Oral antidiabetics and insulin: Beta-blockers may decrease tissue sensitivity to insulin and inhibit insulin secretion e.g. in response to oral antidiabetics. Atenolol has less potential for these actions. The heart rate may not be reliable as an early warning of hypoglycemia. Side-effects: The combination of amlodipine and atenolol is well tolerated. Side-effects include headache, palpitations, flushing, edema, depression, dizziness, dyspepsia, dyspnoea, muscle cramps, fatigue, cold extremities and bradycardia.

Pharmaceutical precautions
Store in a cool and dry place, protected from light.
