Losa
Description:
Losa (Losartan Potassium) is an angiotensin-II receptor (type AT1) antagonist. Angiotensin-II is a potent vasoconstrictor, the primary vasoactive hormone of the renin-angiotensin system and an important component in the pathophysiology of hypertension.

Composition:
Losa-25 : Each film coated tablet contains Losartan Potassium USP 25 mg.
Losa-50 : Each film coated tablet contains Losartan Potassium USP 50 mg.

Pharmacology:
Losartan and its principal active metabolite block the vasoconstrictor and aldosterone-secreting effects of angiotensin II by selectively blocking the binding of angiotensin II to the AT1 receptor found in many tissues, (e.g., vascular smooth muscle, adrenal gland).

Indication/Use:
Losa (Losartan Potassium) is indicated for the treatment of hypertension, congestive heart failure and renal arterial stenosis. It may be used alone or in combination with other antihypertensive & diuretic agent.

Dosage Guideline:
The usual starting dose is 50 mg once daily. In patients with possible depletion of intravascular volume, patients with a history of hepatic or renal failure and in elderly patients of 75 years, the dose may be started with 25 mg. The total daily dose ranging from 25 mg to 100 mg, if blood pressure is not controlled by Losartan alone a diuretic may be added in a low dose. It may be administered with or without food.

Side Effects:
The side effects experienced with Losartan are usually mild and occational. Dizziness is the main side effect with Losartan. Other side effects are hyperkalaemia, diarrhoea, myalgia, urticaria, pruritus, taste disturbance etc. may occationally occurs.

Contraindication:
Losartan Potassium is contraindicated in patients who are hypersensitive to the active ingredient or any component of the drug. It is also contraindicated to pregnant woman.

Drug Interaction:
No drug interactions of clinical significance have been identified. Compounds that have been studied in clinical pharmacokinetic trials include Hydrochlorothiazide, Digoxin, Warfarin, Cimetidine, Ketoconazole and Phenobarbital.

Use in Pregnancy & Lactation:
Pregnancy: Losartan must be discontinued as soon as possible when pregnancy is detected.
Lactation : It should not be prescribed during lactation, as there is no information in human on the passage of Losartan into breast milk.

Over Dose:
The most likely manifestation of overdosage would be hypotension and tachycardia. Bradycardia could occur from parasympathetic (vagal) stimulation. Supportive treatment should include repletion of the intravascular volume. Neither losartan nor the active metabolite can be removed by hemodialysis.

Precaution:
In patients who are intravenously volume depleted (e.g. those treated with high-dose diuretics), symptomatic hypotension may occur. These conditions should be corrected prior to the administration of losartan or a lower starting dose should be used. A lower dose should be considered for patients with a history of hepatic impairment. Losartan should not be used with potassium-sparing diuretics.
